Please use this form to anonymously inform the OMG Ombuds of an uncomfortable or unprofessional situation. General comments and questions should instead be sent to [email protected].

When writing your description, please enter as much relevant information as you can. This includes persons involved, date(s), time(s), and a brief summary of the event or events.. As this is an anonymous reporting channel, the Ombuds cannot follow up with you, but action will be taken as necessary.
